Personal Health Information Audit Policy

Annually, in September, StayWell Custom Communications will conduct an audit of our privacy policies and procedures.

The Audit is intended to assure a) that there has been no material change in the use or disclosure of personal health information and access to that information. In the event there has been a change, the audit needs to assure that policies and guarantees to members who input personal health information have been updated to reflect those changes.

The Audit will also review all disclosures of Personal Health Information over the course of the previous 12 months to make sure that each instance followed all public guarantees of privacy and all internal policies.

This Audit will be conducted jointly be the vice president /editorial for StayWell Custom Communications and the chief technology officer for StayWell Interactive. Results of the audit and any recommendations for changes in policies and procedures will be sent to the Quality Oversight Committee for its approval.
